Here is what you can expect at your first visit to Lee Dental:
After scheduling your appointment, you will receive a link to complete your new patient paperwork online. Please bring your driver’s license and proof of insurance with you to your visit. Upon arrival, you will be greeted by our office staff. Please arrive 15 minutes before your appointment to allow time for any additional steps.
If you need assistance with the forms, please let us know and arrive at least 20 minutes before your scheduled appointment to receive help.
